Transcript Request Form

Transcripts are available for former students of Tabernacle Christian School. Please complete and submit the form below to obtain a copy. Transcripts will be provided in person or mailed to secondary or post-secondary institutions. ID must be shown at the time of receipt. Please call the school office at (828) 324-9936 to confirm your transcript is ready for pick-up. If you would like your transcript sent to a secondary or post-secondary school, please indicate that on the form below. If you require your transcript to be mailed to you personally, you will be contacted to answer additional security questions over the phone prior to release.
